This visit may be appropriate if:

• You are stable on your medication

• No dose changes are needed

• The medication is not controlled

How it works:

1. Submit a brief medical update.

2. Your information is reviewed by a licensed Family Nurse Practitioner.

3. You receive guidance and a personalized care plan.

4. If additional evaluation is needed, your provider will outline next steps.

This visit is not appropriate for:

• Controlled substances

• New medication starts

• Complex medication management
